LINK benefits families, doctors and other health care providers, schools, health departments and other agencies that help protect our community from disease.

How does LINK benefit my children?

  • Your child's vaccination history will be easily accessible by a new doctor's office
  • New yellow cards can be printed at any time
  • Eligibility for enrollment in schools is easy to prove
  • Extra shots due to lost documentation are no longer needed

How much does it cost?
LINK is free. There is no charge for a child to be included in the immunization registry. You will only have to give your verbal consent for your children's immunization information to be included in the registry.

Do I have to participate in the registry if my provider asks?
Parents have the option to refuse to have their child's information entered into the registry. You must simply alert your physician that you do not want to participate. If you change your mind after refusing, you have the right to request that your child's information be included in the registry.

What are my rights?
You have the right to:

  • Look at your or your child's shot record and report any mistakes
  • Find out who has looked at your or your child's shot information through this computer system
  • Refuse to allow shot information to be shared through the computer system. If you wish to refuse, please ask the clinic staff for a refusal form.

Who can my doctor or nurse share the shot information with?
Your doctor or nurse may share the information about your or your child's shots with other doctors and nurses who give you or your child medical care, and local and state health departments. Some information also can be shared with agencies including WIC, schools, childcare facilities, family childcare homes, and healthcare plans who need to know about the shots you or your child received. The information can be shared only with agencies allowed by the California Health and Safety Code.

What can the users of this computer system use the shot information for?
The information can be used to see if you or your child have all the shots that are needed, to give the right shots at the right time, to let you know when you or your child need a shot, and to bill your insurance company. The information may not be used for any other reason.

What information can my doctor or nurse share with other users of this system?
This is a list of the information that your doctor or nurse can share:

  • Name
  • Date and place of birth
  • Current address and telephone number (this information will be shared only with doctors and nurses who give you or your child medical care, and local and state health departments).
  • Parent or guardian's name.
  • Immunizations (shots) received.
  • Health problems you or your child may have had after getting a shot.
  • Other non-medical information needed to correctly identify your or your child's shot record.

How can I participate?
Ask your doctor. All Los Angeles and Orange County physicians - and their patients - are welcome to participate in LINK.
